The 兔子先生 is planning for school-based COVID-19 vaccination clinics for students in partnership with the 兔子先生 County Health Department and Whitney Young Health Services.
To help our partners plan for the best locations and the number of sites needed, we are asking families of children age 5 and older to complete a short survey. The survey will take about a minute. There is no login required.
We will provide more information about vaccination clinics after collecting your feedback.
